What to Expect in the Fourth Trimester: Navigating the First 12 Weeks After Birth

You’ve spent months preparing for birth, but what about life after baby arrives? The first 12 weeks postpartum—often called the fourth trimester—is a time of deep healing, emotional shifts, and round-the-clock newborn care. It’s beautiful, messy, exhausting, and magical all at once.

As a postpartum doula, I’ve walked alongside many new moms in this season, and I want you to know: you’re not alone in how you feel. Here’s what to expect and how to make this transition smoother.

1. Your Body is Healing (Give It Time!)

Your body just did something incredible—whether you had a vaginal birth or C-section, recovery takes time. Expect:
Afterbirth contractions (they help your uterus shrink back down)
Vaginal soreness or incision tenderness
Bleeding (lochia) for up to 6 weeks
Hormonal shifts leading to night sweats, mood swings, or feeling weepy

💡 What helps: Rest as much as possible, nourish yourself with warm, healing foods, and ask for help when you need it!

2. Sleep Will Be...Different

Newborns sleep a lot (around 16-18 hours a day), but it’s broken up into short chunks—meaning your sleep will be too. The biggest challenge? They need to be fed every 2-3 hours, day and night.

💡 What helps:
✔️ Sleep when you can, even if it’s just a nap
✔️ Consider overnight postpartum support (that’s where I come in 😉)
✔️ Let go of the pressure to do “all the things” and focus on rest

3. Breastfeeding Can Be Harder Than Expected

Breastfeeding is natural, but that doesn’t mean it’s easy. The first few weeks can bring:
Latch struggles (it takes practice!)
Engorgement or sore nipples
Frequent feedings—cluster feeding is common!

💡 What helps:
✔️ Work with a lactation consultant or doula if needed
✔️ Try different breastfeeding positions to find what’s comfortable
✔️ Stay hydrated and well-fed—your body needs fuel!

4. Your Emotions Might Surprise You

One minute you’re overwhelmed with love, the next you’re crying over spilled milk (literally). This emotional rollercoaster is normal—hormonal shifts, exhaustion, and the weight of new responsibilities can feel intense.

💡 What helps:
✔️ Lean on your support system—partner, friends, doula, therapist
✔️ Talk openly about how you’re feeling—your emotions are valid!
✔️ Watch for signs of postpartum depression or anxiety (you don’t have to suffer in silence)

5. You Deserve Support—Don’t Do It Alone

Many new moms feel pressure to “bounce back” or handle everything alone. But the truth? You weren’t meant to do this by yourself. In many cultures, new mothers are surrounded by support—help with meals, baby care, and healing.

💡 What helps:
✔️ Say YES when someone offers to bring food or hold the baby
✔️ Consider hiring a postpartum doula (like me!) for extra care
✔️ Give yourself grace—your only job is to heal, bond, and adjust
